Initial Consultation
For your first visit, you’ll need to fill out and bring in the forms you've downloaded from this site. During our first session, we’ll discuss any current complaints you’re having, including any current or past medical conditions, and provide you with a thorough nutritional analysis & ultimately formulate a wellness plan to support your health goals.

This initial consultation lasts about 60 minutes. We will review the information that you supply from the following forms. Download Intake Forms

Food Journal: A 3 day food diary will provide us what you eat and give some insight into how food affects your energy levels.

Health History Form: Please complete this basic demographic form as thoroughly as possible.

Systems Survey Form: This is a very detailed questionnaire that will give us a great deal of information about your symptoms and concerns.

If you are not eating enough healthy fat, you could be depriving your body from these important fat-soluble vitamins. Start by having 1 avocado a day. Other healthy fat sources are coconuts, raw butter, and extra virgin cold pressed olive oil.
